Problem solved after vet visit revealed UTI and adding a second litter box

The Problem

My 4-year-old cat has started peeing on the couch, the laundry pile, and next to the litter box but not in it. This started about a month ago and it's getting worse. The house smells terrible and I've ruined two cushions. The litter box is clean โ€” I scoop it daily. I love this cat but I can't live in a house that smells like cat pee. I don't know if it's behavioral or medical.

The Plan

Week 1: Rule Out Medical Issues First

  • Take the cat to the vet ASAP โ€” inappropriate urination is the #1 sign of urinary tract infections, crystals, or kidney issues
  • This is not optional: medical causes are the most common reason and they're treatable
  • Bring a urine sample if you can (your vet can tell you how to collect one)
  • If the vet clears medical issues, then it's behavioral โ€” but always check medical first
  • Don't punish the cat for peeing outside the box โ€” they're not doing it to spite you, they're communicating something is wrong
  • Week 2: Fix the Litter Box Setup

  • The rule: one litter box per cat plus one extra. One cat = two boxes in different locations
  • Try a different litter โ€” some cats develop preferences or aversions. Unscented clumping litter is safest
  • Make sure boxes are in quiet, accessible locations โ€” not next to the washing machine or in a tight corner
  • Clean soiled areas with enzymatic cleaner (Nature's Miracle) โ€” regular cleaners don't remove the scent and the cat will return to the spot
  • Try an uncovered box if you're using a covered one โ€” many cats feel trapped in covered boxes
  • Week 3: Address Stress Factors

  • Has anything changed recently? New pet, new person, moved furniture, construction? Cats are sensitive to change
  • Add Feliway diffusers โ€” synthetic pheromones that reduce stress-related behaviors
  • Ensure the cat has vertical space (cat tree), hiding spots, and a predictable routine
  • If you have multiple cats, make sure there's no bullying around the litter box
  • If the problem persists after medical treatment and environmental changes, ask your vet about a veterinary behaviorist referral
  • Resources

  • Nature's Miracle enzymatic cleaner โ€” the only thing that truly removes cat urine odor
  • Feliway diffusers โ€” clinically proven to reduce stress behaviors in cats
  • r/CatAdvice โ€” community help for cat behavior issues
  • ASPCA behavior helpline โ€” free advice for pet behavior problems
  • Follow-Up Result

3 weeks in: vet found a urinary tract infection โ€” antibiotics cleared it up in 10 days. Also added a second litter box in the bedroom and switched to unscented litter. Cat hasn't peed outside the box since the UTI was treated. The enzymatic cleaner saved the remaining couch cushion. Added a Feliway diffuser as a precaution and the cat seems generally calmer. Lesson learned: always go to the vet first. What looked like a behavior problem was actually a medical one. The cat was literally trying to tell me something was wrong.
